I do not have my medical records me. How can I get it?
Call us and we can send you a release form so we can obtain your medical records from your provider.
I have anxiety and PTSD and goes to therapy but but do not have medical records about it. How can still be qualified to get an mmj card?
Our office can do a mental health evaluation for you to establish your diagnosis which you can use as a record to get qualified for the program.
